    At the Haven today ~ 4:30pm, 04/06/2024, I asked for a next of kin form to change my next of kin for a more appropriate person. I was told there is no such form and I could write-in and they would change it on-line. I said "I know that a next of kin form exists, I have seen one before" but the receptionists said "we don't have one, "I will email the practice manager to see if he can help". This is a ridiculous situation that is potentially putting Patient health at risk.

    At the Haven today ~ 12:15pm, 06/06/2024, I asked for the second time for a next of kin form to remove a next of kin. I was told "there isn't a form to remove a next of kin, the next of Kin Form can only be used to add an extra next of kin and I could write-in and they would change it on-line". I know that a next of kin form can be used to remove a next of kin, I have seen one before but the receptionists was adamant. This is a ridiculous situation that is potentially putting Patient health at risk.

    On viewing my medical record today, I found that my medical Data has been changed and edited on a number of occasions, it has not been kept private.

    Telephoned surgery, offered telephone consultation with GP as I wasn’t sure a face to face visit was necessary. . Had appointment within a couple of days. GP suggested I needed examination. Appointment made within a few days. Friendly welcome from reception staff, thorough examination by GP, treatment plan explained and exercise details sent to my phone. Can self refer to MSK if no improvement. No waiting around for appointment, everything explained. The staff are doing a great job and as a patient I really appreciated all they do.
